Navigating the Patient Portal
Enter your first name, last name, and DOB in MM/DD/YYYY format.
Then you will need to accept the Terms and Conditions and click 'Submit'. To do so, scroll all the way through the Terms and Conditions; you will not have the option to select the checkbox until you have.
If you are having trouble scrolling through please try accessing your imaging on a computer rather than a phone or tablet.
If you see this message that your imaging was not found, it may be because the institution entered your name in a different format. See table below for examples:

If you still cannot sign in, please contact Ambra Support using the in app widget in the lower right hand corner of the page, or call us at 888-315-0790.
From there you have the option to send a PIN to either your phone or your email address. You should have given this information when you had your imaging taken, so if you do not have the option you're looking for please reach out to Ambra Support. Please do not navigate away from the PIN entry page while you retrieve your PIN.
Once you have received the PIN, enter it in the required field. Click 'Submit' and you will be able to view your imaging and reports.
